What are some plausible ways a doctor would want to persue a physical relationship with a patient?

I'm wondering what are some plausible ways or reasons that would make one or two doctors pursue a physical relationship with a patient. Is there a way for this to be done without playing out like a fantasy?

This happened to one of my friends.  One of their parents who has an oncologist fell in love with a patient who was going through chemotherapy and this resulted in a divorce.  My understanding is that it happened over a long period of time, and I imagine the parent was already unhappy in their language.  I could imagine spending a lot of time with someone when emotions are running high could result in an affair.
