Hi,

If the agent's name is: Mr. First Name Last Name, how do I address him?

1) Dear Mr. First Name Last Name,

2) Dear Mr. Last Name,

3) Dear First Name Last Name,

I've read somewhere that the third option is the most popular, but I haven't written to any agent yet. I would use the third option :)

I agree; I've used both a first name and a first/last name. I've yet to get one of those agents, so...

Most agents accepting submissions are young, so just First Name is fine. I haven't met many in this business who are old-fashioned.

As long as you get the name right, I think you're fine.

I address all agents with First/Last name. Then again, I think I'm an old-fashioned old man from the 1800's.
